From Examination to Intervention: Clinical Reasoning in Complex Neurological Cases
Complex neurological patients rarely present with a single isolated deficit. Dizziness, imbalance, cognitive complaints, autonomic symptoms, visual disturbances, and movement abnormalities often reflect dysfunction across multiple interacting systems. In these cases, effective care depends on more than identifying abnormal findingsā€”it requires determining which findings are clinically meaningful and how they should guide intervention.
This case-based presentation will explore a practical framework for neurological clinical reasoning, moving from history and examination through functional hypothesis development, treatment selection, and reassessment. Using examples from patients with concussion, vestibular dysfunction, persistent dizziness, and other complex neurological presentations, we will examine how oculomotor, vestibular, postural, autonomic, and sensorimotor findings can be integrated to develop individualized rehabilitation strategies.
The emphasis will be on clinical reasoning rather than protocol-based treatment, with the goal of providing attendees with a framework they can apply to neurological patients in everyday clinical practice.

Dr Derek Barton
Dr. Barton earned his Doctor of Chiropractic degree from Life University, where he graduated with Clinical Excellence honors. He subsequently completed postgraduate training through the Carrick Institute, earning Diplomate status with the American Chiropractic Neurology Board (DACNB) and Fellowship status with the American Board of Brain Injury and Rehabilitation (FABBIR).
Since joining Plasticity Brain Centers in 2015, Dr. Barton has focused his clinical practice on the assessment and rehabilitation of patients with complex neurological conditions. His clinical interests include concussion and traumatic brain injury, persistent dizziness and vestibular disorders, dysautonomia, movement disorders, and other complex neurological conditions.
His approach emphasizes detailed neurological examination, objective assessment of oculomotor, vestibular, autonomic, and postural function, and the application of individualized, intensive neurorehabilitation strategies.
